Automatic Survey for voice application

Hi all,
it's possible to invoke a Voice Survey automatically when agent close the voice interaction? we need to know if it's possible to transfer a voice interaction to a specific flow when agent close the voice interaction.

We need a specific action script on customer side in that case?
